As children mature into adulthood, learning how to manage their own heart health is critical to their future wellbeing. The Congenital Heart Transition Program NYU Langone,, a collaboration between the Pediatric Congenital Heart Program and the Adult Congenital Heart Disease Program, teaches children how to take charge of their own cardiac care and connect them with experts in adult congenital heart disease.
Helpful Tips
The transition process starts at age 12 and is complete by age 21 or shortly thereafter, when the child has fully transitioned to the Adult Congenital Heart Disease Program.
